What are the sections of the DLA?

As of July 2010 there were 21 sections to the DLA form, under the following headings.

Moving around outside (this is for the higher rate of mobility component)
Having someone with you outside (lower rate of mobility allowance)
Getting in and out of bed (care allowance)
Help with your toilet needs (care allowance and perhaps mobility)
Washing, bathing and looking after your appearance (care)
Getting dressed or undressed (care allowance)
Moving about indoors (both allowances)
Falls or stumbles (an important section for those with cataplexy)
Mealtimes (an often misunderstood section)
Medication (care allowance)
Communication with other people (care)
Help you need when going out (mobility)
Someone keeping an eye on you (this is about the supervisory section of care)
Dizzy Spells, blackouts, fits or something like this (again important for cataplexy)
The way you feel because of your mental health (the only section specifically about mental health)
Preparing a cooked meal for yourself (again often misunderstood and very important)
When you are in bed (night time care needs)
More about the way your illness affects you (again an important section)
When your problems started (at least six months ago!)
A statement from the person who knows you best and
Additional Information

The form might not say these specific words in each section, but that, at the moment is what each section seems to be about.
