Hi

Advice or some information needed.

i have finally had my blood test back which test for b12, thyroid, kidney function etc
all my test came back fine so the doctors can not find the problem for my extreme tiredness however he has now prescribed me Provigil (Modafinil) i started taking it a week ago, i took one every morning for the first three days the first morning wen i took one my mum n dad in the afternoon noticed the difference i went to sleep n only slept an hour usually i would sleep couple of hours n i came downstairs n felt awake. On the 3rd day the doctor sed to up my dose to 2 tablets one in the morning and one at noon. I have slept diffrerent than i usually do but i dnt feel tht much different i still feel tired and wanting to sleep

Does any one know how long it shud take for these tablets to work?
should i feel different by now? i got to go n see my doctor again tn 2 weeks to discuss how ive been gettin on with the tablets
should i consider a higher dosage? there has defantly been some improvement in my tiredness

Hello Kathryn and welcome to the forum

Good news about your tests but was that where they stopped? Have you been referred to a sleep clinic for fresh tests to see if you have narcolepsy? If not you really do need that to be done. On the about narcolepsy section above you can find details of your nearest unit. Provigil (or modafinil) should only be prescribed to those with narcolepsy.

I do keep saying this but here we go again; narcolepsy is a spectrum dissorder which means that while all people have the same sort of symptoms these vary from patient to patient and so the control varies and the dose varies. Provigil was considered the "step change" in narcolepsy medication and for many people it has changed their lives while for others it didn't work at all. It does take time to work though and certainly weeks not days should be given before you give up on it. Sad to say you might feel different by now but also you could feel for the same for a couple of weeks and then it just pings into place. A higher dose is always possible; Provigil is considered a safe and well tested drug (it has been around for over 25 years) so far higher doses than you are on could be considered.

i have undertaken sleep tests to test for narcolepsy n they cudnt find any evidence of it and he did no further tests or anythin else to help me so i was quite disappointed with that he didnt take it any further. So i had to go back to the doctor to say what now as i dnt have narcolespy and he sent me for those blood tests which like i sed came bk fine. Even tho these tablets r for people tht have narcolespy i am so happy i have been prescribed something that might help me stay awake throughout the day even if it means i only sleep half an hour in the day wud be an improvement! i didnt ask the doctor how long it takes for these tablets to work as i didnt relli want to knw because i wanted to feel different on my own n not wait for wen they shud be working if tht makes sense i have felt abit different since taken them but i still need my sleep in the day. Everyone is different so might take a couple of weeks like you sed for me to relli feel the difference.

Hello Kathryn - I was speaking to a Sleep Doctor last evening and he suggested that if anyone was having symptoms which thier doctor did not find conclusive then keep a diary of when you sleep and how you sleep and go back armed with this information and specifically ask to be referred to a sleep centre. If you have uncontrollable urges to sleep then include this in your diary too. It does take a little while to get a clinic appointment and they first assess then test you - it may take a 2-3 months in total but stay with it and things do get better once your medication is sorted.

Kathryn, can I make a suggestion(and I might be completely wrong)? Other things can cause this overwhelming tiredness.

Your thyroid tests came back "normal". Unfortunately this normal range is just a average normal. For most people ( I presume) if they are above the top of the scale they are underactive and if below, overactive. (Yes, the usual test range is that way round) But not everyone fits this average- I used to work with someone who had blood levels which would leave me never getting out of bed yet she was at work, and with my blood levels she would probably have been bouncing off the walls. OK , we were at either extreme but we demonstrate my point- a doctor should consider symptoms, not solely blood tests.

Google "underactive thyroid" and see if the symptoms sound familiar. If so, go back to your GP and ask to be referred to an endocrinologist . If not, you have only wasted a few minutes on your computer.
