Sometimes you may need a doctor’s letter as evidence of your diagnosis of narcolepsy, for instance to support an application for benefits.
To make it as easy as possible for your doctor to provide a suitable letter, hopefully without charge, we have prepared a template that you can download and print. Just add your details, and ask your doctor to sign and add their name and their surgery’s rubber stamp. The doctor can also indicate whether they consider the letter to be valid indefinitely, or just for a limited period of five years.
